Mariam Al-Khouri leads Northline Clinics Group, a network of primary and specialist outpatient clinics operating across Dubai and Sharjah. She trained as a physician, practised for six years, then moved into health system management after concluding that the constraints she found most frustrating in clinic were administrative rather than clinical.

Growth at Northline under her leadership has been steady rather than dramatic, and she has resisted the acquisition-led expansion common in the sector. Her stated reason is that clinical quality systems do not transfer with a signature: a newly acquired clinic inherits a logo immediately and an operating standard slowly, and the gap between those two events is where reputational damage lives. Northline instead opens sites directly, staffs them from an internal pipeline, and accepts a slower footprint.

She has invested heavily in the parts of care delivery patients notice and boards rarely discuss: appointment scheduling, insurance pre-authorisation, the legibility of an invoice. The group publishes internal waiting-time data by site and has tied a portion of management compensation to it, a mechanism she describes as imperfect but honest.

Al-Khouri is a persistent advocate for better continuity of primary care in a market shaped by high population mobility and employer-linked insurance. She has argued that a health system in which most residents change providers whenever they change jobs will structurally under-invest in preventive medicine, and that the fix is administrative rather than clinical.

She sits on a regional clinical governance committee, supervises trainee physicians one morning a week, and has said she keeps clinical hours specifically so she cannot lose the argument with her own doctors about what is practical.
